Welcome to our firm.....

We would like to thank you for allowing us to help guide you with your tax and financial needs. At Cheryl A. Alleman, CPA, PA, we pride ourselves on delivering the highest quality personal service to our clients. In order for us to be prepared for your appointment, we would appreciate you taking the time to complete the information below. Upon completion, please hit the "Send to Cheryl A. Alleman, CPA, PA" button at the bottom of the page. This information will be forwarded securely to our office and we can record your information in our computer system ahead of time. This will expedite your appointment so that we can spend as much time as possible with you.

When you come in for your appointment you will need to bring the following:

  1. Copy of your driver's license(s).
  2. Social security card(s) for all dependents on your tax return.
  3. Copies of your last (2) two year's tax returns.
 

If you have a situation that requires us to contact any government agency, you will be required to sign a Power of Attorney (POA) so that we may discuss your tax account with the agency. This POA only allows us to discuss your tax account with the respective agency.

If we need to contact your previous CPA or tax preparer you will be required to sign a letter of release authorizing the predecessor preparer to release any information we may require to complete your tax returns and to inform us of the status of their business relationship with you. This is to aid us in having the "big picture" to resolve any outstanding issues that may need to be addressed.

Our fees are based upon the services provided and time spent on the project. Our fee is due in full when billed and prior to any return being e-filed or picked up. If any bill is outstanding beyond 30 days a 1.5% per month finance charge will be due.
